Please advise your clients to see their General Practitioners (GP)’s for a referral letter to see Dr. Dang Nguyen, as this will enable your clients to receive a Medicare rebate.
If you would like to provide additional information to Dr. Dang Nguyen to help with the care of your client, please email your own letter to Health Wise Innovations along with a signed Consent Form from your client to release information to Dr. Nguyen as well as to allow Dr. Dang Nguyen to communicate with you. This will also enable collaboration between Dr. Dang Nguyen and yourself in the care for your client.
